In the same way, donors have the right to donating their organs and should consent to such action without which it would not be executed. Statistics indicate an increasing gap between the number of organ recipients and organ donors. As observed by Brezina (2010), the number of organs from the dead and living donors falls far below the number of patients waiting for transplant. Because of this high demand, there has been a rise in the black market business for human body organs. Kerridge et al. (2002) observe that the high prices that these organs fetch encourage nurses and other healthcare practitioners to remove organs from dead bodies, even where there was no consent to do so, for their own good, giving the example of a kidney which would cost between $ 1,000 and 3,000 in the black market. I believe that if diversity within workforce is present, there is absolutely no place for racial discriminations. Workplace environment must constantly encourage ethical practices. It is imperative that managerial leadership must display high standard of code of conduct because they are people who are capable of motivating other for optimal work performance. Indeed, the leaders, through self example and having exemplary traits like tolerance, cross cultural understanding and empathetic attitude hugely create an encouraging work environment. They also promote positive relationship that reflects in improved productivity an d good will of the organization. I was offended by the whole episode and reacted by pointing out that the manager was not only violating work ethics but that his act can be justifiably construed as racial discrimination. As a junior manager, my comments were not well received but atleast the young man was allowed to serve new customers. I was given a negative entry in my employee logbook.

Taxanes and anthracyclines are the antineoplastic drug groups most commonly implicated. Nail changes can involve several or all 20 nails and appear in temporal relationship with drug intake. Most of the time, toxicity is just a matter of concern due to cosmetic reasons, however very occasionally it may be associated with pain. True incidence of nail changes related to particular drugs are still lacking due to under-reporting and ignorance of potential side-effects among the patients, necessitating the education prior to institution of drugs and appropriate management like, avoiding potential irritants and use of topical, or oral antimicrobials, and possibly cessation or dose reduction of the offending agent or use of cryotherapy in some cases may certainly be beneficial. Introduction Chemotherapeutic drugs are associated with variety of nail changes. These nail changes are usually temporarily and use to resolve once chemo toxic drugs are stopped. Common drugs affecting are hydroxyurea, cyclophosphamide, vincristine, doxorubicin, dexamethasone, methotrexate and cytarabine. We here report our experience with two patients who developed nail changes during the course of chemotherapy for hematological malignancies Case Report 1 A 19-year-old young boy was admitted to the our hospital’s hematology ward with the symptoms of fever and lymphadenopathy in cervical and axillary region .Workup including bone marrow examination, PET CT scan, excisional biopsy of lymph node were done and diagnosis of T cell rich B lymphoblastic lymphoma STAGE IIIB was made .He received 1st cycle of RCHOP while hospitalized and later on discharged .He received rest of seven RCHOP courses on outdoor basis. Through examination during his outdoor follow up visits revealed transverse hyper pigmented bands on the nails (chromonychia). The bands were multiple, transverse, approximately 1-mm broad, spanning the entire nail breadth, dark grey colored, nonblanchable, nonpalpable with smooth overlying nail surface on the nail plates of all fingers and toes with well appreciation of intervening faintly grey colored bands of nails(Figure 1,2). Unluckily, he relapsed post eight cycles of RCHOP chemotherapy and henceforth started on 2nd lin e chemotherapy RICE (Rituximab, ifosfamide, cisplatin, etoposide), 1st course of which was given to him uneventfully recently till the time of writing this report. Case Report 2 A 55-year- old female presented to hematology clinic with complaints of pain abdomen for 6 months with history of significant loss of weight and appetite. Per abdomen examination revealed presence of abdominal lump with dimensions of 18 cm x 15 cm with no hepatosplenomegaly.Tru-cut biopsy of lump was done .Histopathological examination and immunohistochemistry confirmed high grade, B cell type non-Hodgkin’s lymphoma. She was further investigated and found to have stage IVB disease .Due to financial issues she was given CVP regimen (cyclophosphamide,doxorubicin,vincristine),however she relapsed following 3 courses by the time of writing this report. During her outdoor visits, she was found to have diffuse, black pigmentation with longitudinal striae in the nails of digits of both hands (figure 3, 4) and feet. Discussion Amongst the innumerable side effects that chemotherapeutic drugs have, mucocutaneous one are the most worrisome for the patients probably due to cosmetic reasons. Different patterns of nail discoloration like chromonychia, leukonychia (including Mees and Muehrckes lines), Beaus lines, paronychia and onycholysis have been well documented(1). Drugs commonly implicated are – vincristine, hydroxyurea, etoposide, daunorubicin, bleomycin, cyclophosphamide, dacarbazine, 5-fluorouracil and methotrexate (1, 2). The most frequent variety of chromonychia is melanonychia which is best defined as a dark pigmentation of nails observed as diffuse, transverse, or longitudinal band patterns(2).Some more common and important nail toxities have been summarized in table 1 with their description of morphology and pathophysiology of origin. Effect of cyclophosphamide on nails ranges from diffuse, black pigmentation to dark longitudinal striae , and diffuse, dark gray pigmentation located proximally, with overlying transverse, black bands(2). Doxorubicin produces commonly transverse, dark brown bands alternating with white stripes and dark brown diffuse pigmentation bands 4–5 mm wide, which affect two-thirds of the distal portion of the nail(3). Hydroxyurea produces pigmentation that has a diffuse, dark brown color which may appear as single or double transverse bands. Our patient received all of the above mentioned drugs during his due course of illness Table1. Definitions of some common nail toxities and probable pathophysiology of origin (1, 2) Conclusion Nail changes although seems to be common following chemotherapy, however goes unnoticed and underlooked in the lights of more important burning issues of chemotherapy both by physician and patient and hence true incidence of nail related changes is much more than reported especially in dark colored individuals like our patient and hence requires keen observation during follow up of these patients. Human Companionship in Mary Shelly’s Frankenstein :: Mary Shelly, Frankenstein

Human companionship is one of the most basic needs of humans that can be seen in the Creation story. It is tricky for any human to find the perfect companion especially if one is one of a kind. In Mary Shelly’s Frankenstein two characters exemplify this need. Dr. Victor Frankenstein and The Creature are in search of companionship, and they will go to great lengths to achieve it. The classic theme of perversion of family is a major component in Frankenstein. Dr. Frankenstein comes from a good family but in his adult life he longs for a new companion this is mainly found in the Creature and Elizabeth. The development for the need for the Creature starts when he falls in love with knowledge and is furthered when he leaves to study. In his child hood he has â€Å"Natural philosophy is the genius that has regulated my fate; I desire, therefore, in this narration, to stat those facts which led to my predilection for that science†(Shelley 36). This passion develops into his obsession in his adult life when he gains more accesses to knowledge and equipment. Then it climaxes with start of the creation of the Creature because his accesses to bodies and tools. (quote Intro) â€Å"I read with ardour those works, so full of genius and discrimination†¦ it easily conceived that my progress was rapid†(48). His description of the creation makes it seem like he is mothering a child into birth. He distorts the sanity of child birth by creating a human in a lab. This also makes him the mother and father of the Creature. (quote intro) â€Å"When I found so astonishing a power placed within my hands, I hesitated a long time concerning the manner in which I should employ it†¦ my first success to permit me doubt of my ability to give live†¦Ã¢â‚¬ (51). This illustrates his power that he has that was never meant to be any humans. With the successful test my can create life, strengthens his bond with this impending birth of Creature, who embodies all of his scientific achievement. (quote Intro) â€Å"How can I describe my emotions at this catastrophe, or how delineate the wretch whom with such infinite pains and care I had endeavored to from?† (55). Frankenstein shuns his own creation, whom he should be the loving parents of. The culmination of all of Frankenstein’s education led to creation his own companionship; he can not bear to see his hideous creation.

The company top executive Gorge E. Willis knew at least 500 employees personally which satisfied the workers need to be recognized. 2. What reco mmendations would you make to Mr.. Willis? I believe Mr.. Willis should continue using the open doors policy and maintain the good relations with employees. In the same time he needs to give some authorities to middle managers to make the decisions they need to keep the work stream flows. He should also focus more on making the products delivered in the right time and fix the delay in delivery.The company strategy hasn't changed for a while, and it is important to make some critical changes in the company environment to attract more employees instead of making the company place the employee go to in order to have high wage only. Implementing new technologies that assure reducing the number of workers will be more efficient. Finally, he can't depend on the employees self observation only, but also needs to have closer eye on them to determine how efficient they are. CASE-8 DIAGNOSTIC PRODUCTS CORPORATION: 1 .Evaluate both the design of the EDP Performance Bonus Program for Us-based f ield service engineers (Fees) as it currently exists and the way in which the program is being implemented. What changes would you suggest, if any? Explain The program were based on the Fees performance in six areas: 1- cross training 2- PM completion 3- teamwork factor for MS 4- complete first visit 5- call back rate and 6- administrative functions. The current system is not affective in evaluating the employees because of many reasons. First, the employees have no complete control in some areas.For example, complete service events in the first day are not always possible no matter of how hard the employee tries because of many factors that are not related to the employee abilities. The word Solution should be understood as mixture and not as answer as we will see later. On October 26, 1984, during the night, John McCollum shot himself in the head with a .22 caliber handgun. He was 19 years old. It was reported that when he entered the room, the coroner found headphones on John's head. He had been listening for most of the evening to Ozzy Osbourne's albums "Diary of a madman" and "Blizzard of Ozz" in the living room. He then went in his bedroom, listened to Osbourne's album "Speak of the devil" and shot himself". The island is roughly one hundred square miles with 124.6 miles of tidal shoreline. General Fitness And Training Physical Education Essay My chosen pursued activity is general fitness which is health-related;specifically circuit training. In pursuing such activity I may improve both my strength and cardiovascular fitness.Circuit training is essentially structured exercise.Aerobic fitness,strength and flexibility are all improved pursuing circuit training.Incorporated into such an exercise structure are fixed weights/machine stations which isolate specific muscle groups.A complete exercise set is achieved within a given period of time,usually 20 minutes.An uninterrupted flow of activity from machine to machine may enable proper gain of aerobic benefit.The heart is pumping at a steady high level. Every gym session will consist of a warm-up with mobility exercises;ending with the cool-down.Each exercise station exercises a different muscle group from the last.The whole session should last one hour.(Pollock et al.,1998) Circuit training should not occur on consecutive days,two or three visits to the gym per week being sufficient.Selection of correct weights,repetitions and positions is necessary to minimise occurrence of injury and to achieve desired fitness objectives. Aerobic endurance is exercised by completing a circuit as quickly as possible.Significant gains may occur for strength,muscular endurance and flexibility.Physical fitness consists of components.These are aerobic endurance,strength,speed,flexibility,muscular endurance,power and agility.Training programmes may be customized to improve on a selection of these.The most important aspects applicable to circuit training are aerobic endurance;muscular endurance,and flexibility. When the body performs for a prolonged period of time with a low work rate aerobic endurance comes to the fore;such a training will condition the heart and lungs to function more efficiently.Choosing a work-out on the rowing machine for some 20 minutes as an example;this cardiovascular ergometer is very effective in establishing a proper base of muscular endurance that initiates improvement in other components of fitness. Muscular endurance is a function of aerobic endurance without whose supply of oxygen it could not rapidly perform.It is defined as a single muscle or group performing recurrently against variable resistance.For example dumbbell(DB) lunges or shoulder press with a barbell(BB).Body resistance circuit training that includes both free and fixed weights is well suited to improve muscular endurance. Usually overlooked is flexibility,the range of motion(ROM) at a joint or series of joints.This component however is vital in the prevention of inadvertent injury.It is sufficient to perform the warm up including mobility exercises(developing a full range of movement[ROM]) and cool down stretches;all being required for a main session of gym activity. My gym circuit therefore is comprised of a warm-up lasting 5 minutes on a suitable cardiovascular machine eg the treadmill, that simulates a walking gait exercise aerobically.This is followed by the set of mobility exercises(not stretches) to loosen my joints and produce more synovial fluid;gently and rhythmically exercising.This is still aerobic.There follows a set of stretches to prepare the main muscle groups of the body for an imminent main activity session.These too are aerobic;not as intense yet steady,controlled,positioned for some ten seconds. The main cardiovascular machine chosen for a full work-out of at least twenty minutes is the rowing machine,which exercises all main muscle groups with the heart as target muscle. A customized programme working the rower will produce an aerobic curve with a rising and falling RPE[rate of perceived exertion 0-10 on the Borg Scale](Gunnar Borg(1973) validated by(Kang et al.2003)also (Steed,Gaesser,and Weltman 1994) Aerobic contribution is present in the Cool down using a different cardiovascular machine followed by stretches including some that are developmental;included to improve flexibility in the bigger muscles eg the hamstrings(ACSM,2006) As I want to improve my strength and endurance it is important that I obtain profile data from the apparatus and exercises outlined above so that I can clearly see whether I am making any improvements from when I began. The Principles of Training should be applicable to every exercise and sporting category.Individuals and teams then have specific objectives and goals to aim for in their training schedules.Else, all would be blindfolded resulting in overtraining,burn out and poor performance results. Demands on the body higher than the norm comprise what is known as Overload that in turn has related factors of intensity(how hard);duration(how long);type(sport/activity);frequency(how often). Frequency is self-explanatory,often resulting in a higher level of performance. As workload steps up so does intensity.Heavier weights,longer stretches.Such results take time.Overload may be achieved with a higher number of repetitions or performing the same with reduced time-spans. The body is a natural adapter to overloading,so training should be progressive to prompt a response.When this occurs improvement is tangible especially at the beginning.As sets increase muscle strength and endurance increase. It is important not to be too slow in progression. Biceps curls for example.If working with 10 lb weights taking it to 3-4 sets at 15 reps before muscle fatigue is experienced then it is better to do 2 sets of 15 lb weights. Specificity of a chosen,pursued activity needs to be understood.What am I training for? This is very important and relevant for strength training.Exercise has to be specific for each muscle group and strength type required.Balance has also to be included and therefore other exercises of a general nature such as the squat provide an excellent base for development of specific exercise.Training must be specific which means that it should concentrate on the particular needs of the individual within the training programme.(Sharkey and Greatzer 1993)For example,lifting weights will increase muscular strength but will have little effect on the aerobic capacity of the individual. Although training should be specific to a sport this does not mean that training for sport will have little effect on another.Transfer of training can take place where the sport or parts of a sport have a great deal of similar elements of fitness are common to many sports. The opposite of Progression is Reversibility.Training and performance when falling off will signal to the body for an appropriate response.Aerobic capacity diminishes rapidly with no exercise(Coyle,Hemmert,and Coggan 1986) also (Saltin et al.,1968) Muscular endurance with muscles no longer used falls away three times more rapidly than when gained.Performance of skills may be affected through physical deterioration(Greenleaf et al.1976) A number of training methods exist designed for the different fitness components.Circuit training may be viewed as interval training containing high-intensity anaerobic periods with weights and low-intensity aerobic periods of recovery.This training method is able to improve specific areas of the body for muscular endurance.A circuit improves both aerobic fitness and strength thereby providing for much needed conditioning.Aerobic training also involves continuous/steady state training(McArdle et al.,2006)Other methods consist of interval(McArdie et al.,2006) and fartlek training.Flexibility training incorporates both passive and active,static stretching;dynamic and proprioceptive neuromuscular facilitation(PNF) stretching. Suitable to my requirements is a muscular endurance circuit.This will enable me to withstand fatigue,hold to a given position,and to perform repeated muscular contractions for a given period of time.Selection of appropriate exercises needs a central focus of balanced muscle groups.Improvement of cardiovascular exercise and muscular endurance exercise may be achieved by alternating them within a circuit programme.The back squat for instance utilises many muscle groups,that work simultaneously.A main cardiovascular work-out,for example, requires at least 20 minutes on the rower, being correctly positioned within the circuit.The remaining floor-based stretches are performed at the end of the gym session.This saves the heart rate from decreasing too much. Progression and overload are important to consider when a circuit training programme is being planned. The principle of overload indicates your body systems must be stressed beyond their normal levels of activity if they are to improve.(Williams 1993:18).Progression can be maintained simply by increasing the number of repetitions per exercise; reducing the recovery period(secs) between each set of exercises;increasing the resistance of the exercises by weight . Stimulation is applied using the principles of overload and progression during circuit training so that adaptation may occur. Overload is delivered by adjusting one or more of the FITT principles.Frequency(how often);Intensity(how hard);Time(how long);Type(suiting sport/activity). Principles are usually installed in most matters and physical activity is no exception. The principles of training are the rules to follow when using physical activity programmes.Sound and useful training takes into consideration all of the principles and their effects on the body;being essential to the planning of the training programme so that the participant is able to improve their fitness level. Fitness levels vary from person to person so the training needs be systematic taking into account individual needs ; variables of difficulty or intensity are set at the personal level. An example from the free weights exercises for progression/adaptation is the Biceps Curl with barbell(BB).Apart from applying a progressively greater weight performance may be effected through different ranges of motion i.e. halfway up and down.All the way up and halfway down;up again and all the way down;all the way up and down.An example of an adaptation for a fixed weight machine is substituting the seated row for the lateral pull down.Again the seated cable row may be substituted by the single arm row with a dumbbell(DB).Or the Triceps pull down (cable) by selection of a Triceps extension with butterfly grip(DB). It may be necessary to increase aerobic fitness and if so, use of one of the cardio-vascular machines is ideal. The rowing machine for example,is an excellent piece of equipment for a good all over workout,impact- free; protecting joints while improving flexibility Using this machine you will work out your heart,lungs,circulatory system and at the same time shape and tone your legs,back,shoulders,buttocks,arms and stomach. If aerobic fitness needs to be increased interval training is very effective when inserted into circuit training.Interval training is a type of physical training that involves bursts of high intensity work.This high intensity work is alternated with periods of rest or low activity,intervals.[ see Appendix] The term can refer to any cardiovascular workout,for example,rowing,that involves brief bouts at near-maximum exertion interspersed with periods of lower-intensity activity.Interval training improves the exercisers aerobic capacity to exercise longer at varying intensities.This method of training may be more effective at inducing fat loss than simply training at a moderate intensity level for the same duration. With no correction or improvement reversibility occurs.Having ceased training the body loses its conditioning and strength and also endurance.This is relevant to myself as I am pursuing a cardiovascular, strength and endurance programme.A study has been made of cessation of physical activity;in this case an Olympic rower.It was 20 weeks before he was able to resume his activity following an eight week convalescence. The Dollmaker by Harriette Arnow The Dollmaker by Harriette Arnow is the story of one woman whose genius is undermined by those surrounding her. Gertie Nevels, a tall, big-boned woman raised in the Appalacian region of Kentucky is creative, self-sufficient, strong, and resourceful. In her native home, Gertie creates for herself an atmosphere where she is able to survive any situation and has everything under control. As Wilton Eckley states in â€Å"From Kentucky to Detroitâ€Å", a chapter in his novel, Harriette Arnow, â€Å"Certainly while the family is living in Kentucky, she [Gertie] is self-sufficient and has no fear that she will be unable to get along if Clovis is called to join all the other younger men of the community in the service† (87). In fact, she is able to make her dream of moving into a larger farm and expand her comforts and resources, along with providing better for her family without the help of her husband; however, when she is forced to move to Detroit, she is out of her element. Throughout her life, she has molded herself to be able to do any task that is stereotypical of males or females. Even though initially she finds it hard to live the â€Å"city life† that is typical of Detroit, she is able to adapt her mind to be able to tolerate- and eventually accept- her new surroundings. Although Gertie Nevels shows obvious genius, because of her geographic disposition and gender, she is unable to use her abilities to the fullest extent. Harriette Arnow uses the first scene in her novel to exemplify Gertie’s strength, intellectual capacity, practical usage of skills and courage; furthermore, she portrays (within the reaction of the soldiers) the reaction that outsiders usually have to her. Because of her dialect a... ...er needs the block of wood to feel as if she can connect to God. Harriette Arnow, in her â€Å"Letter from a Writerâ€Å", states that â€Å"Gertie split her wood because she didn’t need it anymore; it was not an act of despair.† Moreover, â€Å"It hurt and was a sacrifice in a sense made for all the things she hated† (25).
